Registration Form for Group Classes

Please complete & submit this form if interested in any of my group yoga and/or Self-Breema® classes.

Unless otherwise noted under a class' details, my GROUP class rates are:

sliding scale per class: $25 to $15
sliding scale 10-class pass (which expires 3 months after purchase): $250 to $120

My PRIVATE* yoga class rates (either 1.25 hours or 1.5 hours in length) can be found Here.
* This is for a class for only 1 student. If you would like to schedule a small, PRIVATE GROUP class which is not open to the public, there may be an additional fee for an additional student.

I accept payment via PayPal or Apple Pay Cash. If experiencing a financial hardship and unable to pay within the sliding scale, please let me know.

To participate in any of my classes &/or other services, please mark the liability waiver box, below, confirming your agreement with it. By submitting this form, you are also acknowledging that you understand & accept: ✨that Soizig Le Boulch (“Soizig”) is a Birth Doula and so may on occasion need to cancel or reschedule a class or appointment with less than 24 hours notice; ✨Soizig’s DONATIONS Policy: Donations I make to Soizig are non-refundable and non-transferable. If I make a donation to Soizig in mutual support for one of her Donation-based classes, I am encouraged to pay Soizig my donation immediately *AFTER* the class; however, in the very low likely chance that I make a donation for that class in advance and Soizig has to cancel it due to a Birth or other special circumstance that class is not available to me, I can apply that donation to another one of Soizig’s classes in the upcoming month; and ✨Soizig’s PAYMENT & CANCELLATION Policy: for private appointments with Soizig, payment to Soizig is due in Full at least 48 hours in advance of the appointment. Soizig accepts payments to her Apple Pay Cash account and to her PayPal account. Cancellations or other requested adjustments to dates and/or times of an appointment must be reported to Soizig (via text at 415-508-7649) or email at [email protected]) more than 48 hours before the scheduled appointment. Your payment in Full is still due to Soizig for any appointment you cancel within 48 hours of the scheduled appointment or to which you are a “No Show” and is non-refundable and non-transferable. EXCEPTIONS: If Soizig is able to accommodate your request made before the appointment to make a small adjustment to the appointment time (such as moving your appointment time 15 minutes earlier or later), Soizig will be happy to do so. In the event of your unexpected illness within 48 hours before the appointment, Soizig will be happy to credit the funds to a future appointment scheduled within the next 2 weeks. [In the event of an unavoidable emergency that requires cancellation within 48 hours, please contact Soizig so that we can come to an agreement that is respectful to both of us and of our respective needs and pre-existing commitments.□️] *

LIABILITY WAIVER: I have read, fully understand, and hereby voluntarily agree to all of the following: ✨1. Risk of Activities & Responsibility for Risk: I am aware that participation in various forms of exercise such as walking, Yoga, Breathing Practices, Self-Breema Exercises®, TRE®, and other practices, modalities, methods, programs, &/or workshops ("Activities") may result in discomfort, injury, &/or other damages and I assume all risk connected with my participation in  any and all  Activities offered by Soizig Le Boulch now and/or in the future. I acknowledge that Soizig Le Boulch has not and will not render any medical services including medical diagnosis and that it is my responsibility to consult with a physician prior to and regarding my participation in Activities offered by Soizig Le Boulch and that I perform Activities within the safety guidelines given to me by medical provider(s), physical therapist(s), etc. I represent and warrant that I am in sufficient health and suffer from no physical, mental, or emotional condition which would limit my safe participation in Activities according to my own comfort and within the safety parameters given to me by my medical (and/or other health) provider. When participating in Activities offered by Soizig Le Boulch — whether with or without Soizig Le Boulch present with me while I am practicing them — I am encouraged to and am responsible for participating with the gentle principles of “Body Comfortable” and “No Force”, by tuning into my body, mind & feelings, and adapting/modifying and/or skipping any of the Activities presented in Soizig Le Boulch's classes, appointments, &/or other events to ensure my comfort, safety, and general sense of well-being at all times. During online classes with Soizig, unless otherwise invited by Soizig, I will keep my Video and Microphone features disabled (set to “off”) to minimize distractions for the other participants. I understand that logistically, when teaching a group class virtually, especially when students’ Video settings are disabled, Soizig will be unable to offer me feedback, so, again, I take full responsibility to participate in class based on my own comfort and enjoyment. If I have a question for Soizig about something she presented in class, I will wait until she has ended the class to unmute myself and ask my question. For an interactive, personalized support from Soizig where I can ask questions and/or receive feedback throughout the session, I can schedule la private 1-on-1 appointment with Soizig.  ✨2. Release of Liability: I hereby, on behalf of myself, my heirs, executors, administrators, assigns, or personal representatives, knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims, or causes of action of any kind whatsoever arising out of my participation in the Activities, classes, appointments, and/or other events with Soizig Le Boulch, and do hereby release and forever discharge Soizig Le Boulch, her affiliates, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical, psychological, emotional, &/or economical injury, loss, or other damages, which I may suffer as a direct result of or otherwise related to my participation in the aforementioned Activities, classes, appointments, and/or other events with Soizig Le Boulch.
